This form is necessary for tax reporting purposes, as sign wavers are classified as independent contractors. The W-9 allows us to accurately report your earnings to the IRS and issue a 1099 form at the end of the year. Please make sure to fill out the W-9 form completely and submit it prior to your first assignment.
