All existing sign wavers must submit their completed time sheet at the end of each day (not weekend!!).

Make sure the sheet has the following information written correctly:

1. Store name, number, and address
2. Date, time in-out, and total time
3. Store rep initials

Incomplete sheets may cause delays in payment, so please double-check all details before submitting.

Your photo must show the ENTIRE SHEET OF PAPER FROM CORNER TO CORNER!
